WINS Health Organization Grant is committed to collecting only information reasonably necessary to administer its programs. The applicant portal does not request Social Security numbers or SSN card images.
Information
The applicant portal may collect your name, full address, email address, phone number, gender, age, date of birth, marital status, hearing status, housing status, occupation, monthly income, household size, requested grant amount, purpose of applying, and optional additional information. This information is used to administer applications and conduct program review.
Sensitive information
Applicants should provide only the information requested by the form. Do not enter Social Security numbers, passwords, bank login credentials, or identity-document numbers.
Security
Application information is stored in the program's application database and access to administrative application records is restricted to authorized administrator accounts.
